ACL Surgery

A tear of the anterior cruciate ligament (ACL) can immediately affect stability, confidence, and the ability to participate in the activities you love. Many patients describe a sensation that the knee “isn’t trustworthy” during pivoting, cutting, or landing. These injuries are especially common in soccer, basketball, football, lacrosse, volleyball, and skiing, but they can occur during any sudden twist or misstep.

Why the ACL Matters

The ACL is one of the knee’s primary stabilizers, controlling rotation and forward motion of the shin bone. When it tears, the knee may give way unexpectedly and become vulnerable to further damage, including meniscus tears and cartilage injuries.

How ACL Tears Happen

ACL tears often result from:
  • A sudden pivot or change in direction
  • An awkward landing
  • Rapid deceleration
  • A twisting fall
  • Direct contact with the knee
A “pop,” immediate swelling, and difficulty bearing weight are common signs.

Dr. Pearle's Expertise

Dr. Pearle is internationally recognized for ACL reconstruction and injury prevention. As a member of the international ACL Study Group, he has shaped modern ACL techniques and outcomes. His approach combines precise surgical technique, individualized planning, and evidence-based rehabilitation strategies.

How the Procedure Works

ACL reconstruction is performed arthroscopically through small incisions. A personalized graft—often hamstring, patellar tendon, quadriceps tendon, or allograft—is used to recreate the ligament in a position that mirrors the natural ACL.
The goal is to restore stability, protect the joint, and support return to full activity.

Recovery Expectations

Recovery is progressive and personalized.
It generally includes:
  • Early focus on motion and swelling control
  • Structured strengthening and balance training
  • Gradual introduction of running and agility
  • Controlled return to sport once key strength and movement goals are met
Patients receive a tailored plan based on their activity level and goals.
